I would stay with "They paid up front and still left" for the week already named. A quieter result is still a result.
What was the first sign you trusted in "The note was polite"?
The refund was the right call if the contract was fuzzy. I would still ask what month two failed to do. Annual customers who leave early are specific, not a trend, until the third one.
